Preparing for a CHC funding assessment
It is important to prepare thoroughly for a continuing healthcare assessment. A multidisciplinary team (MDT) will be looking at health needs across 12 'domains' as well as factors such as intensity, complexity and predictability of those needs.

While decision support tools (DSTs) are designed to help the MDT correctly identify a primary health need, some decisions are subjective and open to interpretation.
